To give us enough time to assess your travel plans and organise any recommended vaccinations, please submit a Total Digital Triage request at least  10 weeks before you travel.

We will then send you a Travel Assessment Form, which should be completed and returned at least 8 weeks before your departure date.

Returning your form early helps ensure you receive the appropriate advice and protection before your trip.

Please note:  We can only book a travel appointment with our Nurses once we have received your completed Travel Assessment Form.

If your form is returned less than 8 weeks before travel, we may be unable to offer an appointment or complete all recommended vaccinations before your departure date.
